If you are feeling unprepared or overwhelmed at the responsibility of caring for your loved one, we provide reassurance and education to help care for them at home. Includes nursing assessment, medication review and reconciliation, care coordination, education and teaching on specific disease processes, and communication with health care provider as needed.

Wellness Check (4 visits/month) $300.00
This is a monthly membership with a goal to keep your loved ones out of the hospital long-term. We will keep an eye on your family member(s) or loved ones and update you weekly as needed. As a member, you will receive the following services:
1 hour weekly in home visit/wellness check (90 minutes for 2 people)
B/P, Temperature, Heart Rate, Oxygen level
Heart, lung, circulation, mentation assessments
Check the home environment for safety
Review any medical reports, upcoming visits and organize them
Medication education, pour medications and make a schedule if needed, and review
Review any medical tests
Education on health issues
Schedule appointments if needed
Communicate with medical team if needed
Weekly updates to family as needed

Post Hospitalization Care Management $150.00

Review of discharge instructions, newly prescribed medication teaching, nursing assessment, education on new or exacerbation of disease process, and care management.
Pre-Op/Post-Op Surgery Consultation and Care Management $150.00
2 hours (1 hour pre-op and 1 hour post-op)
We will help you prepare for your upcoming surgery by assessing your goals and needs as well as providing pre-op education. Includes a home visit to assess home safety and readiness. We ensure you understand all your pre-op instructions.
We will review what to expect after surgery. We will review you diet and ensure you are providing your body with the nutrition it needs for optimal healing and speedy recovery. When you return home after surgery, we will help you and your family understand your discharge instructions including post surgical care, assess your vital signs, monitor for post-op complications, and provide education for pain management and new medications prescribed.
Additional services and visits can be arranged if needed. Changes to above plans can be made to fit your individual needs.
